How Acupuncture Reduces Pain

In order to feel pain, there are two types of nerves involved.

The first type are nociceptive nerve fibres which send signals to the brain if there’s pain. There are again two types of these: A delta fibres (think sharp, burning pain) and C fibres (dull throbbing pain).

These nerves send signals to the spine up to the tract of Lissauer and to the brain. 

The second type of nerve involved in pain are proprioceptive nerves, which tell the brain where the location of pain is.

They send signals from the affected area, up the spine to the brain. The result is the brain responds by releasing endogenous opioids (pain killers) that bind to pain receptors along the spine and the capillary beds where pain exists. This reduces pain. 

For patients with pain that has continued for more than 6 months, the signal produced from the proprioceptive nerves is usually weak, meaning the signal to the brain is weak.

With a weak signal strength, the brain doesn’t get the message to release pain killers, causing pain to linger and turn chronic. 

Acupuncture is used to stimulate and “wake-up” the proprioceptive nerve pathway. The sensation created by the needles ‘jump starts’ the neural signal threshold and stimulates the brain to release enkephalins once again. 

The good news is that after a series of visits, the body can re-establish the normal threshold to signal the brain.

This means the patient will be less reliant on drugs to deal with the pain because the body’s self-pain regulating mechanism has improved. 

Example

Imagine you hurt your knee. Your brain doesn’t know what you did to hurt your knee; it just senses the pain. The brain doesn’t know whether you fell, were stabbed or poisoned.

In response to pain, the brain doesn’t want whatever happened to your knee to put the rest of your body at risk (it doesn’t want anything to spread).

So, the brain sends a signal to clamp down and tighten the small blood vessels and muscles in the area in order to sacrifice the knee to protect the rest of the body.

Two things happen when we insert an acupuncture needle.

First, we’re stimulating a nerve. Our goal is when we stimulate that nerve, the signal travels up that nerve into the spine and to the brain. This jump starts the brain to release its own pain killers. 

Secondly, when we take away those pain signals, the brain accepts the knee is now safe.

This results in the widening of small blood vessels around the knee, allowing fresh blood to push back into the damaged area.

The damaged area now has direct access to the resources it needs to heal. That is blood (Xue), oxygen (Qi) and nutrient (Ying Qi).
